Lab Card and Lab Card Select Programs
Save on outpatient lab work when you use your Lab Card or Lab Card Select. The Lab Card and Lab Card Select are innovative programs from Quest Diagnostics with more than 3.4 million members nationwide.
The Program is Easy
Members show their medical ID card with the Lab Card/Select logo at the doctor's office when laboratory work is required and request that their specimens be sent to the Quest Diagnostics approved facility for testing. With Lab Card, the member pays nothing for laboratory testing -- no deductible, no coinsurance and no copay. With Lab Card Select members receive significant savings for specimens sent to the Quest Diagnostics approved facility. If the doctor customarily collects specimens in the office, he/she simply sends those specimens to Quest Diagnostics.

How to Use Your Lab Card
- When your doctor orders laboratory work for you, show your medical ID card with Lab Card/Select logo and tell them that you would like to use the benefit.
- Your doctor's office collects your specimens and calls the testing facility for pickup.
- The testing facility performs the tests and sends the results to your doctor (usually within 24 hours).
