Trustmark Affinity Market's Claim and Eligibility Lookup provides our members online access to claim payment status as well as eligibility/benefit information for themselves and their family members.

After you register online to use the Claim and Eligibility Lookup, simply enter your Social Security number and Group ID number, located on the front of your medical ID card to access your secure claims records. Through the site, you can view each family member's claim status, eligibility, individual and family deductible and out-of-pocket amounts that have been satisfied.
