ID Card Request

Employers and members with insurance through Trustmark Affinity Markets can request additional or replacement ID cards online whenever needed. 

If you have used any of Trustmark's Web-based systems, such as Claim and Eligibility Lookup, you can proceed directly to the ID Card Request link below. You will be asked to do or confirm the following:

  • Type of card(s) needed — Medical, dental or both
  • Verify correct address
  • If information is incorrect, you may select RESET to make any needed corrections.
  • Hit SUBMIT to complete and process the request.

If you have not used any of Trustmark's Web-based systems previously, click on "ID Card Request" below, and then click on "Register Here" to follow the prompts to register with Trustmark. After you have registered, you can request ID cards.

** Please allow seven to 10 business days for your new ID card(s) to be mailed. You may not place another order for the same ID cards within two weeks of the first order.

ID Card Request
