Physician/Hospital Lookup

Use the pull-down menu to access a list of managed care networks associated with our medical plans, including Aetna Signature Administrators^SM, a national network with more than 793,000 healthcare professionals nationwide. Choose your network from the list to locate a participating physician and/or hospital in your area. If you are unsure which network you belong to, look at the logo on your medical ID card.

If you will be traveling outside of your standard PPO network, you can search for a PHCS Healthy Directions physician or hospital. Then select "Search for a Provider," and choose Healthy Directions.

If your provider is not currently participating in the network and you would like him or her to join, please have the provider contact the network to begin the application process. To be considered for most networks, the provider must have admitting privileges at a participating hospital, meet the network’s credentialing criteria and accept the network’s discounted rates.
